Aged Care Case Manager

An aged care case manager coordinates and arranges care for older adults. They assess the needs of their clients, develop personalised care plans, and connect them with appropriate services such as healthcare, home support, and social activities. Their role is to ensure clients receive comprehensive care tailored to their needs, enhancing their well-being and quality of life.

Support Coordination

A support coordinator helps individuals with disabilities navigate and manage their NDIS plans. They assist in understanding and implementing the plan, linking participants to service providers, and ensuring that the support meets the participant’s goals. They can assist in monitoring the participants budget and work on a support plan for any complex needs. They also handle any issues that arise to ensure services are delivered effectively.

Disability Consultant

A disability consultant provides expert advice, strategies and plan to improve accessibility and inclusion for people with disabilities. They work with the person to link them to organisations, governments, and individuals to design and implement services that accommodate people with disabilities, ensuring quality of life and inclusive living within their community. This can be booked through our subscription service when you sign up. We offer two types of subscription packages.

Basic

Subscription will give you unlimited access to the resources and information online but also will give you one consultation a month with one of our consultants or advocates to discuss all your queries and concerns.

Advocacy for participants

At Unleashed Futures, we are committed to forging partnerships with various Advocacy organisations to strengthen our initiatives in addressing the diverse challenges and needs faced by individuals with disabilities. We will assist you in assessing whether you are encountering difficulties with Self Advocacy, Individual Advocacy, or Systemic Advocacy, and we will focus on connecting you with the appropriate organisation tailored to your unique situation. Our goal is to empower individuals with disabilities by offering valuable resources, support, and connections that enable them to effectively advocate for themselves and actively engage in the decision-making processes that impact their lives. By fostering these relationships, we strive to create a supportive environment where every individual’s voice is heard and respected.

At Unleashed Futures, our team consists of independent consultants and dedicated advocates who specialise in addressing your housing challenges and necessities. We are committed to offering unbiased advice and personalised support, ensuring that your choices and interests always come first.
Our role involves assisting you in managing and resolving any conflicts of interest that may arise, including those that might simply be perceived. We work to help you understand all available options, providing clarity in complex situations and ensuring that your decision-making process remains transparent and informed.

Advice for Providers

Our independent consulting service specialises in navigating situations where conflicts of interest arise within organisations. We understand that these conflicts can hinder decision-making for participants and compromise integrity and trust with the service provider. Our team of consultants provide impartial assessments and tailored strategies to identify and address actual, perceived and/or potential conflicts. By fostering transparency, we help organisations align their objectives with ethical standards, ensuring that all parties’ interests are managed fairly and equitably. Despite their best intentions, providers are more prone to overreach when participants lack trusted informal supports like family, friends, or advocates and depend on staff as their primary or sole source of information. This risk is heightened for those residing in provider-managed environments, such as Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), or group homes. Additionally, participants with extra communication or supported decision-making needs may find it challenging to convey consent or dissent. Those with a history of trauma or instability may exhibit high compliance and minimal resistance, further increasing the likelihood of overstepping by providers. Ultimately, our goal is to empower participants in supported decision making and control and for organisations to make informed decisions through good governance, processes, policies and information sharing while maintaining their reputational integrity.
