  • Advocate or emergency contact’s telephone number and email

Participant personal information may be collected from:

  • You, the participant

  • Your family members or significant others

  • Your advocate

  • Your doctor or other service providers or facilities

We collect personal information directly from you unless:

  • You have given consent for us to collect it from someone else

  • We are required or authorised by law to collect it from another person

  • It is unreasonable or impractical to do so

You may withdraw your consent at any time by contacting us. Please note that withdrawing consent may impact our ability to provide services.


 
Purpose of Collecting Personal Information

Personal information is collected to provide care and services. It may be used to:

  • Provide support services

  • Enable service providers and medical practitioners to deliver care and services


 
Disclosure of Personal Information

We may disclose personal and health information for care and service purposes to:

  • Service providers assisting in delivering care and services

  • Medical practitioners

  • External health agencies (such as ambulance services and hospitals)

  • The National Disability Insurance Scheme (NDIS)

  • Other relevant government organisations

We may also disclose information to a person you have nominated as your advocate, such as:

  • Parent, child, or sibling

  • Spouse or relative

  • Household member

  • Guardian

  • Enduring power of attorney

  • Emergency contact (must be at least 18 years of age)


 
Use and Disclosure Limitations

We will not use or disclose personal information for purposes other than providing care and services unless:

  • You have given consent

  • The purpose is related to providing care and services and you would reasonably expect the disclosure

  • Disclosure is necessary to prevent or reduce a serious and imminent threat to life, health, or safety

  • Disclosure is required or authorised by law due to suspected unlawful activity

We do not disclose personal information to overseas recipients.


 
Security of Personal Information

We take reasonable steps to protect personal information from misuse, loss, unauthorised access, modification, or disclosure.

  • Secure cloud-based systems

Employee records related to the employment relationship are managed in accordance with workplace laws.

Privacy laws may apply if employee information is used for purposes unrelated to the employment relationship.


 
Volunteer Records

Personal information relating to volunteers is managed in accordance with the Privacy Act.

If personal information is lost, stolen, or accessed or disclosed without authorisation, we will follow our Management of Data Breach Policy and Procedure.

Privacy complaints can be lodged through our complaint handling process.

All complaints will:

  • Be treated seriously

  • Be handled as promptly as possible

  • Be managed confidentially

  • Not affect existing obligations or commercial arrangements

 

You will be informed of the outcome once the investigation is complete.
